NHS Spine Surgery Wait Times and Your Options

Back and leg pain from a spine problem is hard to live with for a few weeks, let alone the months a referral can take. Many people on the NHS find that the wait is not one queue but several: a GP, then a musculoskeletal (MSK) service, then a spinal team, then the operation.

In one line

England has no separate spine figure, but in the orthopaedic and neurosurgery lists that include it, only 60.9% and 62.7% of patients had waited under 18 weeks in June 2026, and Canada's median referral-to-treatment wait is about 49 weeks.

Waits vary by area and by stage of the pathway. Private care at home, existing insurance or surgery abroad can shorten the wait, but each costs money, and after major spine surgery you need weeks before a long flight home.

Here is what the official figures say, why they are harder to read for spine surgery than for most operations, and the realistic options if you cannot wait.

How long is the wait for spinal surgery on the NHS?

Often many months, but there is no national figure for spine surgery alone. NHS England counts spinal surgery inside trauma and orthopaedics, and some spinal work also sits under neurosurgery. At the end of June 2026, the trauma and orthopaedics list held 829,590 waiting pathways and only 60.9% had waited less than 18 weeks, against a standard of 92%. For neurosurgery, 55,909 were waiting and 62.7% were within 18 weeks.

Across all specialties, the median wait was 11.9 weeks and 105,711 people had waited more than a year. Spine patients sit in two of the slower specialties, and the clock covers the whole pathway from referral, so a wait to see a spinal surgeon and a further wait for the operation both count.

How long is the wait for herniated disc surgery or a discectomy?

It depends mostly on how long it takes to see a spinal surgeon, which varies a lot by area. Patients describe waits of several months just for a first specialist appointment, before any date for the operation is discussed.

If your symptoms get clearly worse while you wait, such as new weakness in a leg or foot, tell your GP or the spinal team, because your priority can change. Numbness around the groin or buttocks, or new problems controlling your bladder or bowel, need same-day emergency care, not a waiting list.

How long is the wait for spinal fusion surgery?

There is no separate published figure for fusion in the UK; it falls under the same orthopaedic and neurosurgical lists above. Fusion is usually planned after a longer period of assessment, so in practice many people wait longer than for a straightforward disc operation. Going private does not always remove the wait either:

How long are spine surgery waits in Canada and Australia?

In Canada, the Fraser Institute's 2025 survey of physicians put the median wait from GP referral to treatment at 49.9 weeks for neurosurgery and 48.6 weeks for orthopaedic surgery, the two longest of all specialties (the overall median was 28.6 weeks). These come from a survey of doctors, not hospital records, and CIHI does not track spine surgery waits.

In Australia, we found no national figure for spine surgery. Across all elective surgery, the median public wait was 45 days in 2024–25 and 6.0% waited more than a year (AIHW). In the ACT, news reports put the median wait for orthopaedic surgery at 169 days, with 26.8% waiting over a year.

How long can spine surgery be delayed with physio and injections?

There is no set limit; it is a decision for you and your surgeon based on your symptoms, your scans and how you respond to treatment. Many people try physiotherapy, pain relief or a steroid injection first, and some never need an operation. Others find the waiting itself becomes the problem:

Whatever you choose, keep a short diary of pain, walking distance and sleep. It helps your team judge whether you are improving, and it is useful if you later seek a second opinion.

What are the options if you cannot wait for spine surgery?

There are three realistic routes, and each has a cost.

Paying privately in the UK

Private hospitals list a lumbar microdiscectomy from about GBP 10,123 including surgery, stay and follow-up, and lower-back slipped disc surgery from GBP 11,533 including the consultation and surgeon's fees (2026). Spinal fusion in a London private hospital starts at GBP 17,300 for hospital charges alone.

Insurance

If you already have cover through work, check your policy's spinal pathway. A new policy will usually exclude a back problem you already have.

Surgery abroad, including China

Abroad, you agree the date with the hospital rather than waiting on a list. We did not find official waiting-time statistics for spine surgery in China, so ask for a proposed surgery date in writing before you book. Cost is where China stands out: Shanghai caps the public-hospital fee for a lumbar fusion operation at CNY 5,571–6,686 (≈ USD 830–1,000), implants for a standard fusion fell to about CNY 4,500 (≈ USD 670) after a 2022 national bulk purchase, and a peer-reviewed study of one Chinese teaching hospital found a median total bill of CNY 11,461 (≈ USD 1,700) for spinal admissions in 2023–24. Foreign patients pay the full list price, and international departments charge more.

The trade-off is time away. After a lumbar fusion, one UK private hospital advises no long-haul flights for 12 weeks, while a medical-travel hospital in Turkey usually clears patients to fly after 10–14 days. Plan for the cautious end and let your surgeon decide. Ask whether you can keep your place on the NHS list while you decide.

Make sure your care can continue at home. Before you leave, ask for the operation note and discharge summary in English (Chinese hospitals issue reports in Chinese by default, so request English when you register) and for copies of your scan images in DICOM format, not only the written report. Your GP and any UK physiotherapist will need these for follow-up.

Before you look for surgery elsewhere

  • Get copies of your MRI images (not only the report) and clinic letters
  • Ask the hospital for a proposed surgery date and an itemised written quote
  • Check whether you can stay on your NHS list while you decide
  • Ask how long the surgeon expects you to stay before flying home
  • Arrange physiotherapy and a GP follow-up for when you return
  • Know the warning signs that need emergency care while you wait
