The same colonoscopy can be free or come with a bill

Under the ACA, in-network screening for adults aged 45 to 75 is covered without cost-sharing, and plans may not charge for removing a polyp during a screening colonoscopy. A colonoscopy for symptoms is not a preventive service, and the American Cancer Society warns that insurers may treat some tests as diagnostic, so you may pay the usual deductible and copay. Ask your insurer how the test will be coded before it is booked.

Medicare pays for screening colonoscopy, but if a polyp is removed you pay 15% coinsurance of the approved amount for the doctor, and in a hospital outpatient department or surgery centre also 15% for the facility. On a high-deductible plan, the 2026 minimum deductible is USD 1,700 self-only and the out-of-pocket maximum USD 8,500, so a diagnostic colonoscopy can be mostly self-paid.

UK and Australia

New private cover may not cover a problem you already have

UK moratorium policies exclude conditions you had in the 5 years before cover starts until you have gone 2 continuous years without symptoms, treatment or advice for them. In Australia, an insurer may impose a 12-month waiting period for a pre-existing condition, meaning signs or symptoms in the 6 months before you joined, and 2 months for most other hospital care.
