People weighing a disc replacement against a spinal fusion usually want the same thing: to stop the pain without ending up with a stiff spine or a run of further operations. One patient, who had a microdiscectomy first and then a disc replacement after the disc herniated again, described the result seven weeks after surgery:

The choice depends on your scans, spinal stability, bone quality and number of levels. Get more than one surgical opinion. China prices quoted here are public-hospital list prices and caps; foreign patients pay in full and international departments charge more.

Then reherniated and spent six months doing PT (and walking and swimming and TCM etc etc etc, I tried it all), threw in the towel and got an artificial disc replacement. Not everyone has that experience, and the right operation depends on your scans, your age, which level is affected and the condition of the joints around it. This article explains the trade-offs in plain terms, what recovery looks like, and why the price of both operations in China fell sharply after 2022. It is general information, not advice for your case.
Is disc replacement better than spinal fusion?
Neither is better for everyone. Disc replacement suits a narrower group of patients, while fusion can treat a wider range of problems. In a fusion, the surgeon removes the damaged disc and joins two vertebrae with a spacer (cage), bone graft and usually screws and rods, so that level no longer moves. In a disc replacement, the disc is replaced with an artificial joint designed to keep movement at that level.
The appeal of disc replacement is that it keeps motion and, in theory, puts less extra load on the levels above and below. The limit is who qualifies. Surgeons usually check whether the small facet joints at the back of the spine are healthy, whether the spine is stable, how strong the bone is and how many levels are involved. People with a slipped vertebra (spondylolisthesis), clear instability, marked facet arthritis or weak bone are more often offered fusion.
Cost does not usually decide it. A US analysis of commercially insured claims from 2017 found a mean total cost of USD 26,819 for a neck fusion (ACDF) and USD 25,954 for a cervical disc replacement, as reported by Becker's Spine. Both outcomes exist, and good and bad stories sit side by side in patient forums:
“My ADR surgery was not successful, back under the knife had a second which was fusion… I only tell people if you're at risk of paralysis (I wasn't) or you simply can't take it anymore than try surgery. Always always get 3-4 surgical opinions.”
The practical step is to ask each surgeon why they recommend one operation over the other for your spine in particular, and to get a second opinion before a decision you cannot reverse.
Questions to ask a surgeon about disc replacement or fusion

- Are my facet joints, bone strength and spinal stability suitable for a disc replacement?
- How many levels would you treat, and what is the plan for the levels next to them?
- Which implant would you use, and can I have its make and model in writing?
- How long will I stay in hospital, and what restrictions will I have in the first three months?
- What would happen if I waited and continued non-surgical treatment?
How long does recovery from spinal fusion take?
Most people leave hospital within days, but the bone takes months to fuse. St John & St Elizabeth Hospital in London describes a stay of one night to a few days after spinal fusion. Acibadem, a Turkish hospital group that treats international patients, gives 3 to 5 nights and says the fusion matures over 3 to 6 months, sometimes up to a year (both 2026). During that time bending, lifting and twisting are usually limited, while walking is encouraged from the start.
Neck operations have shorter guide figures in NHS leaflets. After an anterior cervical fusion (ACDF), Robert Jones and Agnes Hunt Orthopaedic Hospital advises at least 4 weeks off work (2024). After a cervical disc replacement, University Hospitals Coventry and Warwickshire gives a hospital stay of 2 to 5 days and no driving for at least 6 weeks. A two-level fusion, older age, smoking and weaker bone generally mean a slower recovery, and your surgeon will set your own limits.
What are the risks of spinal fusion, and what are the alternatives?
Fusion is major surgery. The general risks include infection, bleeding, blood clots, nerve injury, the bones failing to join and pain that continues after surgery. A longer-term concern is adjacent segment disease, where the levels next to a fusion wear faster because they take on extra movement. Disc replacement was developed partly to reduce that risk, but it does not remove it.
“It's a complication that the spine surgeon explained to me before we even decided on a plan… I had specifically asked him if I could basically sandwich my cervical fusion with disc replacement… He told me that even if we do that, there is still a risk of adjacent segment disease. And once you start messing with the spine, it can increase the risk of needing further surgery later.”
The alternatives depend on the problem. For a herniated disc pressing on a nerve, a microdiscectomy or an endoscopic discectomy removes only the piece causing the pressure and fuses nothing. For pain without clear nerve compression, people usually try physiotherapy, exercise, injections and pain management first. Fusion for back pain alone, without a clear cause on the scan, is an area where surgeons disagree, so a second opinion is especially worthwhile there.
Is minimally invasive or endoscopic surgery an option, and does it cost more?
It is an option for some problems, and in China's public hospitals the endoscopic version of a disc operation does not carry a higher surgical fee. Minimally invasive fusion uses smaller incisions and tubes to place the same kind of implants, and endoscopic discectomy removes disc material through a tube with a camera. Whether either suits you depends on where the disc fragment sits and what else needs fixing.
Shanghai's 2026 public-hospital price list, published by the Shanghai Healthcare Security Bureau, states that routine endoscopic disc surgery is charged at the same service fee as open surgery, with no separate charge for the endoscope. China is also active in this field: of 642 research papers on percutaneous endoscopic lumbar discectomy published from 2013 to 2022, 393 came from China, according to a bibliometric study in the Journal of Pain Research (2023). That measures research output, not the number of operations or their results.
What do fusion and disc replacement cost in China?
The implant part of the bill fell sharply after China's 2022 national purchasing round for spinal implants, and public hospitals cap the surgeon's service fee. In the national tender held on 27 September 2022, spinal implant prices fell by an average of 84%, according to China Daily. The tender covered cervical and thoracolumbar fusion systems, vertebral augmentation, endoscopic discectomy products and artificial discs. A standard thoracolumbar fusion implant set fell from an average of about CNY 33,000 (≈ USD 4,950) to about CNY 4,500 (≈ USD 670). Separate average prices for artificial discs were not published, so we do not quote one.

The whole bill is more than the implant and the service fee, because anaesthesia, the bed, drugs and imaging are charged on top. The best whole-bill figure we have comes from one tertiary hospital in Shandong, where the median total cost of a spinal surgery admission fell from CNY 18,288 (≈ USD 2,750) to CNY 11,461 (≈ USD 1,700) after the implant tender (Frontiers in Public Health, 2026). That figure pools all spinal operations, including cheaper ones, and is the total before insurance pays anything. Foreign patients have no Chinese insurance and pay the full list price, and international or VIP departments charge more than the public price list. Ask for a written estimate that names the implant and lists what is included.

Does spinal hardware need to be removed after fusion?
Usually not. Screws and rods are normally left in place for life once the fusion has healed. Removal is a separate operation, and surgeons usually consider it only when the hardware causes a specific problem, such as pain clearly linked to a prominent screw, loosening, breakage or infection. Removal carries its own anaesthetic and surgical risks and does not always relieve pain, so it is a decision for imaging and a surgeon's examination, not a routine step.
If you have surgery abroad, ask for a written record of exactly what was implanted, including the manufacturer, model and size, and keep it with your scans. A surgeon at home who later reviews or removes the hardware will need it, and so will radiology staff before an MRI.
