Types of Rhinoplasty: Open vs Closed, Filler and Cartilage

Choose the approach for your nose, not the label: open and closed rhinoplasty have similar reoperation rates, filler only adds volume and wears off, and breathing problems need work on the inside of the nose.

Have breathing problems assessed before surgery, and ask exactly what your new nose will be built with: your own cartilage, an implant, or both.

Rhinoplasty covers several different operations, plus a non-surgical option that borrows the name. Which one suits you depends on what you want changed, whether you also have trouble breathing, and what your nose is built from. This guide explains the main types in plain terms and the questions to ask, whether you have surgery at home, in Korea, in Turkey or in China.

Is open or closed rhinoplasty better?

Neither is better in general. They are two ways of reaching the same bone and cartilage, and the right one depends on your nose and on what your surgeon does most. In closed rhinoplasty all the cuts are inside the nostrils. In open rhinoplasty there is also a small cut across the strip of skin between the nostrils, so the skin can be lifted and the tip cartilages seen directly.

Open surgery gives the surgeon a clearer view, which is why it is often chosen for complex tip work, very crooked noses and revisions. Closed surgery leaves no outside scar. On the question people worry about most, a systematic review of 49 studies found reoperation rates of 2.73% after primary open rhinoplasty and 1.56% after closed, with no significant difference between them (International Archives of Otorhinolaryngology, 2016).

You will also see newer labels. Preservation rhinoplasty lowers a bump by moving the bony and cartilage roof of the nose down instead of removing it, and suits some noses better than others. Ultrasonic or piezo rhinoplasty uses a vibrating instrument to shape bone. These describe tools and methods, not a guarantee of a better result. A more useful question for any surgeon is which approach they use most, and why it suits your nose.

Is nose filler a good alternative to rhinoplasty?

For small changes, sometimes. Filler, often called a liquid or non-surgical rhinoplasty, can smooth a small bump, raise a low bridge or lift the tip slightly, and hyaluronic acid filler can be dissolved if you do not like it. It cannot make a nose smaller, narrow a wide tip, straighten a bent septum or improve breathing, because it only adds volume.

Filler in the nose is not risk-free. The nose has small blood vessels that filler can block, which in rare cases damages the skin or affects eyesight. That is why it should only be injected by a doctor who treats noses often and can dissolve filler quickly if something goes wrong. Results are temporary, so repeat treatments add up over the years. If you are offered a permanent or semi-permanent injectable for the nose, ask exactly what the product is and whether it can be removed.

What makes a rhinoplasty result look natural, and what is the nose built with?

A natural result is one that fits your face and still works, and it depends as much on skin and structure as on the surgeon's taste. Thick skin hides fine detail in the tip; thin skin shows every edge. A nose that is reduced too much can look pinched and breathe badly, which is why surgeons often add support while they take away size.

That support comes from grafts or implants. Cartilage from the septum inside the nose is used first when there is enough. Ear cartilage is soft and curved, and is mainly used for the tip and nostrils. Rib cartilage is stronger and there is much more of it, so it is used for bigger rebuilds and many revisions, but it needs a small cut on the chest. In East Asia, including China and Korea, raising the bridge with a silicone or ePTFE implant is common, often combined with cartilage in the tip. Implants are simpler to place but can show through thin skin or shift over time, so ask how removal would work.

In China this combination of bridge and tip work is usually called a combination rhinoplasty (in Chinese, 鼻综合). Aesthetic surgery prices in China are set by each hospital rather than nationally, so the figures below are examples, not a price list.

Does rhinoplasty improve breathing problems?

It can, if the cause is structural and the surgery is planned for it. Work on the septum and the inside of the nose is often combined with reshaping the outside, which is usually called septorhinoplasty or functional rhinoplasty. Cosmetic rhinoplasty is not available on the NHS, but the NHS says it may be offered if it is needed for breathing. In Australia, Medicare rebates apply only to documented functional or breathing cases with a GP referral.

Surgery can also make breathing worse. The NHS lists temporary breathing difficulty as a common effect and permanent breathing problems among the serious risks. If breathing matters to you, have it assessed by an ear, nose and throat (ENT) doctor before choosing a surgeon, and ask how the plan protects the airway.

In the UK, five large providers listed septorhinoplasty at about GBP 5,000 to 9,000 in 2026, against GBP 4,000 to 7,500 for cosmetic rhinoplasty (TreatCompare, 2026).

Can a crooked or broken nose be straightened with rhinoplasty?

Usually it can be improved a lot, but perfectly straight is not guaranteed. A crooked nose almost always involves bone, cartilage and the septum together, and bent cartilage can slowly drift back towards its old shape. Surgeons may reinforce or replace it with grafts to hold the new position.

Ask your surgeon how much improvement is realistic for your nose, and look at photos of similar noses a year or more after surgery. Remember too that uneven swelling makes many noses look crooked for months, so the final straightness is judged late.
