Revision Rhinoplasty: When, Rib Cartilage, Risks and Cost

Most noses never need a second operation; if yours does, wait until it has fully settled, usually about a year, and choose a surgeon who does revisions regularly.

Revisions are harder than first operations, often need rib cartilage, and a second revision is more likely after a revision than after a first surgery.

Revision rhinoplasty means operating again on a nose that has already had surgery, to fix breathing, a shape that did not turn out as planned, or a nose that has weakened over time. It is a bigger decision than the first operation. This guide covers how common it is, when it can be done, what the nose is rebuilt with, the risks, and how to choose who does it.

Is it common to need revision surgery after rhinoplasty?

A minority of people have a second operation, but it is not rare, and the chance is higher after a revision than after a first surgery. A systematic review of 49 studies found reoperation rates of 2.73% after primary open rhinoplasty and 1.56% after closed rhinoplasty (International Archives of Otorhinolaryngology, 2016). A large US study of 175,842 patients followed for at least three years found revision in 3.1% after a first septorhinoplasty and 11.0% after a second one (JAMA Facial Plastic Surgery, 2016).

These figures count operations, not unhappiness. More people are unsure about small details, especially while the nose is still swollen, and many of those concerns fade as it settles. Common reasons for an actual revision include blocked breathing, a nose that has stayed crooked, a tip that is too pointed or too upturned, a bridge that has collapsed, or fullness above the tip once the swelling has gone.

How long after rhinoplasty can a revision be done?

Usually not until the nose has fully settled, which in practice means waiting about a year. The NHS says swelling can take up to 6 months to fully go, and hospital patient guides say the final result can take up to a year (Acibadem, 2026). Operating on a nose that is still changing makes it hard to know what needs fixing, and the tissue is still healing.

There are exceptions, such as a clear structural problem that is obvious early on, and that is a decision for the surgeon after an examination. If you are unhappy at three or four months, it is still worth booking a review so the concern is recorded and photographed. It is not a reason to rush into a second operation.

Is rib cartilage better than ear cartilage for revision rhinoplasty?

For a larger rebuild, rib cartilage is usually preferred because it is stronger and there is much more of it. Ear cartilage is often enough for smaller tip or nostril grafts. Revisions often need more support than first operations, partly because cartilage from the septum may already have been used or removed the first time.

Rib cartilage has its own trade-offs. It needs a cut on the chest, which adds pain and a small scar, and grafts can bend slightly as they heal. A nose rebuilt with rib often feels firmer and takes longer to settle. Some surgeons use donor rib cartilage instead of your own, so ask which is planned and why.

What are the risks of revision rhinoplasty?

The risks are the same as for a first operation, and the surgery is usually harder. The NHS lists temporary breathing difficulty, numbness and bruising as common, and permanent breathing problems, cartilage damage, a changed sense of smell, heavy nosebleeds, infection and blood clots as serious risks. Scar tissue from the first operation makes the structures harder to work with, swelling can last longer, and the skin can have a weaker blood supply after previous surgery.

The higher revision rate after a second operation, 11.0% in the US study above, is the clearest sign that each extra surgery is harder to get right. If your current nose works well and the issue is small, a good surgeon may advise you to leave it.

Should you go back to the same surgeon or see a different one?

Either can be right, but get at least one independent opinion before deciding. The original surgeon knows exactly what was done, and some surgeons offer a reduced fee for their own revisions. A different surgeon makes sense if trust has broken down, if the problem is complex, or if the original surgeon does few revisions.

Whoever you choose, ask for a copy of the first operation notes, ask how many revisions the surgeon does, and look at revision results a year or more after surgery. If the first operation was abroad, plan how follow-up will work before you book anything.

How much does revision rhinoplasty cost, and can it be done in China?

Revisions cost more than first operations almost everywhere, and China is not the cheap option compared with Korea or Turkey. In the UK, five large providers listed revision rhinoplasty at about GBP 5,500 to 10,000 in 2026.

In China, aesthetic surgery prices are set by each hospital, and official price lists include separate add-on items for repeat surgery and for grafting your own tissue. Public hospital lists exclude anaesthesia, tests and materials, so ask for a full written quote based on your own photos and scans. Chinese rules require the doctor leading aesthetic surgery to be a registered physician with at least six years of clinical work in aesthetic or plastic surgery, and the hospital to hold a medical institution licence. Ask to see both.

A revision trip should be at least as long as one for a first operation, since the splint and stitches still come off at about one week. Some people use the stay for a general health checkup at the same hospital.

Before you book a revision

  • Wait until the nose has settled, usually about a year, unless your surgeon advises otherwise
  • Get a copy of the first operation notes
  • Have your breathing assessed if it has changed
  • Ask how many revisions the surgeon does and see results a year after surgery
  • Ask whether cartilage will come from your septum, ear or rib, or from a donor
  • Get a written quote that lists anaesthesia, tests, materials and hospital nights
  • Plan to stay until the splint is off and your surgeon clears you to fly
