Most people who plan a knee replacement abroad ask two questions first: how long will I be in hospital, and when can I fly home? The first answer is short. The second is not, and it matters more for anyone travelling to China from the US, UK, Canada or Australia. Here is how one family described the first days after a knee replacement in Shanghai:
In one line
Expect a few days in hospital and about 6 weeks before you walk without an aid, but UK hospitals advise waiting around 12 weeks before a long-haul flight, so plan your stay in China in weeks and let your surgeon decide when you fly.
Timelines vary with age, fitness and the operation. An earlier flight may be possible for some patients with clot prevention and a fit-to-fly letter, but only on your surgeon's advice and within the airline's rules. Arrange physiotherapy and follow-up at home before you travel.
“My mother had her knee replacement in Shanghai last June. She was up and walking after two days and discharged after five, then rested and did her rehab exercises at home. No after-effects, no infection, and she has been fine ever since… It still comes down to the hospital and the surgeon.”
This guide sets out the usual recovery milestones, what they mean for the length of a trip, and where the honest uncertainties are. It is general information. Your own surgeon's plan comes first.
How long does recovery take after a total knee replacement?
Most people are up and walking with a frame or crutches before they leave hospital, can usually walk without an aid at around 6 weeks, and return to work between 6 and 12 weeks, according to the NHS knee replacement recovery page. Full recovery takes much longer. Swelling, stiffness and tiredness often continue for months, and many patients describe the first year as a gradual climb rather than a fixed finish line.
“I had mine done when I couldn't walk 2 miles, or stand for an hour. My life felt limited and small. I'm 12 weeks post op, and I can now walk 2 miles… It's a long recuperation, around a year, although obviously you start to be able to do things a bit at a time.”
The NHS milestones below are a useful general frame. They are not a promise. Age, fitness before surgery, other health conditions and the exact operation all change the pace, and some people move faster or slower than this.
Typical knee replacement milestones (UK NHS guidance)
| Milestone | Usual timing | Source |
|---|---|---|
| Leave hospital | 1 to 4 days after surgery (UK) | NHS |
| Stitches or clips removed | After about 10 days | NHS |
| Compression stockings | At least 6 weeks for most people | NHS |
| Follow-up appointment | About 6 weeks | NHS |
| Walking without an aid | Try after about 6 weeks | NHS |
| Driving | Wait at least 6 weeks | NHS |
| Return to work | Usually 6 to 12 weeks | NHS |
| Long-haul flight | About 12 weeks advised; surgeons' advice ranges from 35 to 180 days | NHS orthopaedic hospital; UK surgeon survey, 2023 |
How long is the hospital stay after knee replacement surgery?
In the UK, people usually leave hospital 1 to 4 days after a knee replacement, according to the NHS. Discharge depends less on a set number of days and more on what you can do: get in and out of bed, walk with a frame or crutches, manage the toilet and, often, a few stairs.
“The hospital will not discharge you unless you walk up a few stairs… You dress, get out of bed and the physical therapists come and show you how to use the walker. You go for a walk with them, and then they show you how to navigate stairs.”
Hospital stays in China tend to be longer. There is no national figure for knee replacement, but a peer-reviewed study at one tertiary hospital in Zhejiang found that hip replacement patients stayed about 6.4 days on average in 2022 (Frontiers in Public Health, 2024). For a visitor, a longer stay is not a bad thing. It means more days with nurses and physiotherapists close at hand before you are on your own in a hotel.
How long should you stay in China, and when can you fly home?
Plan for several weeks, not days, and treat the flight home as a decision your surgeon makes with you. This is the part of a knee replacement trip that is least standardised, so it is worth being clear about what the sources say.
- An NHS orthopaedic hospital (Robert Jones and Agnes Hunt) recommends waiting until 12 weeks after surgery before a long-haul flight. It notes that the risk of a blood clot is higher with poor mobility, dehydration or sitting still for more than 3 hours.
- HCA Healthcare UK says it is usually recommended to wait at least three months before flying after a hip or knee replacement, and that airlines may ask for a fit-to-fly letter.
- A survey of UK surgeons published in 2023 found that advice for long-haul flights (over 4 hours) ranged from 35 to 180 days, with a median of 90 days. Of the 14 airlines that replied, 5 said they would not carry passengers after a joint replacement and 7 required a doctor's certificate.
A flight from China to any of the four countries is long-haul. Put simply, the most cautious advice would mean staying around three months, which few people can do. Some surgeons will clear an earlier flight for a fit patient, usually with blood-clot prevention and a fit-to-fly letter, but that is an individual medical judgement and not something any clinic or agency should promise in advance.
In practice, a sensible minimum plan covers the hospital stay, removal of stitches or clips (after about 10 days, according to the NHS) and a check with the operating team before you leave. Beyond that, ask your surgeon before you book anything: what flight timing they would accept for you, which blood-thinning medicine and stockings they recommend, and whether they will write a fit-to-fly letter. Then check the airline's own rules.
This is also where China is not the easiest choice for everyone. For many Americans, Mexico is a short trip away, which makes the flight home a smaller issue. The trade-off is worth weighing honestly against cost and other factors.
How long does physical therapy take, and what helps before surgery?
Rehabilitation starts in hospital and usually continues for weeks to months, with exercises done at home every day between sessions. The NHS advises that most people wear compression stockings for at least 6 weeks and have a follow-up appointment at about 6 weeks.
Preparation before surgery matters too. Patients often say that the stronger and more active they were going in, the easier the first weeks felt. A physiotherapist can give you exercises suited to your knee before you travel. If you are going abroad, the most useful preparation is to arrange your physiotherapy and a doctor at home in advance, because the bulk of rehabilitation happens after you return.
Before you book flights for a knee replacement in China
- Ask the surgeon how long they expect you to stay in hospital and in the city afterwards.
- Ask what flight timing they would accept for you, and whether they will write a fit-to-fly letter.
- Ask which blood-clot prevention they use (medicine, stockings) and how long it continues after you leave.
- Check your airline's rules on flying after joint replacement.
- Book accommodation with a lift or few stairs and a shower you can step into.
- Arrange physiotherapy and a doctor at home who will see you after you return.
- Get copies of the operation note, implant details and X-rays to take home.
How soon can you drive after a total knee replacement?
The NHS says to wait at least 6 weeks before driving after a knee replacement. You need to be able to control the car safely, including an emergency stop, and be off any medicine that makes you drowsy. Some people return sooner and some later, and the side of the operation also matters for drivers of manual cars. Ask your surgeon, and check with your insurer before you drive again.
For a traveller, the same rule applies to getting around after you land. Arrange for someone to collect you from the airport and drive you to early appointments.
Hip and knee replacement at a Chinese public hospital, after national implant price cuts
For people in the US, UK, Canada and Australia facing a long wait or a five-figure bill for a new hip or knee. China’s 2021 national procurement cut the average price of a hip or knee prosthesis by 82%. At two public tertiary hospitals studied in 2022, the whole hospital bill averaged about CNY 26,365–26,530 (≈ USD 3,950) for local patients. Foreign patients pay in full and there is no official price for them, so every quote is individual.
