IVF Without Insurance: Real Costs and What Coverage Pays For

Two couples can go through the same IVF cycle and pay very different amounts, and insurance is usually the reason. One US patient noticed how large the gap can be:

This guide explains what insurance and public schemes usually cover in the US, UK, Canada and Australia, what IVF costs when you pay yourself, and what to expect from insurance if you are considering treatment in China. It covers standard IVF and ICSI for legally married couples only.

Does insurance cover IVF?

Sometimes, and it depends far more on where you live and who provides your plan than on the treatment itself. In the US there is no federal IVF coverage. Fifteen states and Washington, DC require IVF coverage as of 2026, but self-insured employer plans, which cover about 65% of insured workers, are exempt from state mandates. Many Americans therefore depend on whether their employer chooses to include fertility benefits.

In the UK, NHS funding for IVF is decided locally. The HFEA, the UK fertility regulator, says some areas fund three attempts, others one, and some none. In 2023, NHS funding paid for 24% of IVF cycles in England, 33% in Wales, 49% in Northern Ireland and 54% in Scotland. In Canada, coverage is provincial: Ontario and Quebec each fund one cycle per lifetime, and British Columbia began one-time funding of up to CAD 19,000 in 2025. In Australia, Medicare pays rebates for IVF with no cap on the number of cycles, about AUD 2,800–3,200 per cycle.

How much does IVF cost with insurance?

With insurance, what you pay depends on your plan's limits, and the most common gaps are drugs, embryo testing, freezing and storage. Before starting, check whether your plan has a lifetime or annual cap, whether drugs sit under a separate pharmacy benefit, how many cycles or retrievals are counted, and whether genetic testing and storage are included.

Public schemes leave gaps too. Ontario's funded cycle does not include fertility drugs, which cost about CAD 5,000 per cycle. In Australia, the average out-of-pocket cost after Medicare is about AUD 5,748 per cycle according to a 2026 comparison of 29 clinics, although another page on the same site gives AUD 6,898. One Australian patient described her total:

How much does IVF cost without insurance?

Without insurance, one cycle typically costs USD 15,000–30,000 in the US, about GBP 6,210 on average at a private UK clinic once drugs and ICSI are added, CAD 15,000–20,000 in Canada and AUD 8,000–15,000 in Australia before Medicare. Several destinations abroad advertise lower prices, and the totals below are typical ranges from cost guides and price lists, not quotes.

Base prices often exclude drugs, so compare full estimates rather than headline figures. A Czech clinic's 2026 list, for example, shows CZK 39,000 for an IVF cycle before drugs, ICSI and freezing.

How much do egg retrieval and embryo transfer cost on their own?

In China, each step of IVF has a published maximum price at public hospitals, which makes the parts easy to see. In Shanghai, from June 2024, egg retrieval is capped at CNY 2,500 (≈ USD 370), embryo culture at CNY 4,200 (≈ USD 630), embryo transfer at CNY 2,500 (≈ USD 370) and the frozen-thawed embryo step at CNY 1,240 (≈ USD 190), with ICSI from CNY 2,800 (≈ USD 420). Drugs, scans, blood tests, freezing and storage are charged on top, which is why a full cycle usually totals CNY 30,000–40,000 (≈ USD 4,500–5,950) (People's Daily, 2024).

In the US, the American Society for Reproductive Medicine figure of USD 12,400 for a base cycle without drugs is the most commonly quoted reference point. Itemised US prices vary by clinic and are not standardised.

Does Chinese health insurance cover IVF for foreign patients?

No. As of May 2025, all 31 provinces in China have put assisted reproduction on basic medical insurance, but that cover is only for locally insured residents. Foreign patients pay the full price, and international or VIP departments can charge more than standard public prices. There is no official price list for foreign patients.

This matters when you read Chinese prices online. Local figures often already reflect insurance: People's Daily reported in 2024 that insurance pays about 55% of total costs in Beijing and 50–70% in Guangxi, and Shenzhen reimburses insured patients up to 90% for eight assisted reproduction items. Shanghai limits insured payments to three per person at a hospital. None of these discounts apply to foreign patients, so use the full public price of a cycle, typically CNY 30,000–40,000 (≈ USD 4,500–5,950), when you compare.

We have not found a reliable answer on whether a short-term visitor can buy Chinese insurance that would pay for IVF, so this needs to be confirmed case by case, and we do not assume it. Whether your own insurer at home would repay any part of treatment abroad is also something to check with them in writing before you go. Tax and benefit rules at home may also treat bills from abroad differently, so check those too.

China is also not open to everyone. Treatment is available only to legally married couples of a man and a woman, who show passports and proof of marriage. Hospitals confirm eligibility case by case before you travel.

How do people pay for IVF when insurance does not cover it?

Most people combine savings, employer benefits, financing and, for some, treatment abroad. A US patient who had several rounds listed what helped most:

If you go abroad, choose the destination for more than price. Mexico is closer for most Americans, and Spain, Greece and the Czech Republic are closer for UK patients. China can make sense for married couples who can stay for several weeks: the UK regulator says a cycle takes three to six weeks, and one cross-border couple treated in Guangzhou spent about three months in the country. Check the visa route for your stay, because short visa-free stays may not cover a full cycle.

Note: Genetic testing of embryos is a separate question from insurance. In China it is allowed only for medical reasons, such as a known inherited condition, and is not offered as general screening.
